Recently, the Swedish elderly care system has been depicted with structural deficiencies such as being fragmented, under-staffed, and un-coordinated.
This fragmentation of elderly care might be a consequence of the division of responsibility as elderly care is divided between municipals and regions but also in relation the privatization of services and multiple care agencies providing different health- and care services.
When an older person with complex care needs such as a person living with dementia is discharged from inpatient care, there is often still a need for support and services from different agencies to cater for these needs.
Different providing health- and care agencies are required by law to have a care plan meeting in order to meet and plan the persons future health- and care services. In this processes the person in concern should be involved.
A citizenship perspective entails the promotion of, dignity, rights and agency of all persons to participate in issues that concerns one’s own health and everyday life.The scoping review intends to contribute with evidence on how different providing agencies – with different responsibilities – collaborate and coordinate health- and care services for older people living with dementia.
Further, also the service user is ought to participate in decisions regarding their future health- and care services – something that might be difficult for a frail older person with complex care needs to accomplish.The aim of this research project is to synthesize existing evidence regarding how different providing care agencies within elder care collaborate and coordinate health- and care services for older people with dementia.The method used for the research project will be a scoping review with a qualitative synthesis following the PRISMA-ScR guidelines and PICO(S) framework.
The assessment of quality will follow the methods suggested by SBU, and the grading of evidence will be conducted by using the Research Pyramid Model.
