Demonstrator of Information and Systems sharing with support of Models and Organisational collaboration.

Purpose and goal:

The goal of the project was to create the conditions for developing a system demonstrator with the aim to support an efficient sharing of health data for the elderly with health care needs with the patient in focus. Expected results and effects:

The project resulted in an analysis of organizational, informational, legal and technical prerequisites for a future system demonstrator for efficient sharing of health data in healthcare. The project´s results will be the basis for a future system demonstrator project. Approach and implementation:

The project has been based on the Swedish ´coordinated individual pla´n (SIP) which according to law must be drawn up for a patient in order to achieve better cooperation between actors in care and social care with the patient in focus. The project has since used various methods to analyze and develop this SIP and its related processes and health data.

In the first place, workshops have been carried out using techniques from service design. Analysis of reports and regulations as well as interviews and meetings have also been carried out.
