Communicactive - Preventive and health-promoting efforts in municipal care with the help of IoT

Purpose and goal:

The aim is, through innovative methods, meet the challenge in an aging population. The project aims for more person-centered way of working that is in line with the changeover to "close care". The goal is increased participation and better health:

1.Increased participation and co-creation in efforts via the introduction of digitally supported care, changed processes and new ways of working.

2.Increased security and independence for the individual through support for health-promoting activities that prevent inactivity, involuntary loneliness, malnutrition and falls. Expected results and effects:

Expected results at the individual level are better health, enabling people to stay in their own homes and increased participation and co-creation, which is also health promoting. Expected results at the organizational level are improved working methods that lead to an improved work environment for home care staff and contribute positively to strengthening the profession of assistant nurses.

We intend to spread the results and thus have potential to, in the long term, with a successful implementation in our and other municipalities contribute on a societal level to more equal health.
