Purpose and goal: ** Denna text är maskinöversatt **

The project aims to strengthen the already ongoing strategic collaboration between Karolinska Institutet/Karolinska University Hospital and Mass General Brigham/ Harvard Medical School by extending it to KTH and creating conditions for short-term exchange of personnel (so-called visiting position) and to launch project with international relevance (REDETERMINE) and national relevance (NETMEE-SVERIGE)

Expected results and effects: ** Denna text är maskinöversatt **

Timely discovery of personal risk factors and warning signs enables prompt action for primary and secondary prevention of CMD, aimed at reducing the likelihood of complications, improving results and long-term health, and improving an individual´s quality of life. Within 2-3-years after the projects end, our ambition is to

start to change the future of individuals in Sweden and the EU by starting to implement in society and in healthcare the developed tools and recommendations that will be tested and validated during the execution of the projects. Approach and implementation: ** Denna text är maskinöversatt **

The first (WP1) aims at short-term exchanges of staff (so-called visiting positions) between KI/K and KTH and MGB/HMS. Exchange should result in better insight on both sides of current data structures, challenges and actor constellations. The other two aim to create two different pilot project (WP2 and WP3) using care-related Real World Data and advanced Clinical Research Data to create AI/ML-based digital tools for personalized prevention and treatment of cardiometabolic diseases in a federated "GDPR- compliant” platform.
